Jon Forché, O.D., F.A.A.O.
DoctorDr. Jon Forché is a graduate of Miami University where he graduated Magna Cum Laude with a B.A. in Biological Sciences. He received his Doctorate in Optometry from the Ohio State University College of Optometry. He completed a residency in Low Vision Rehabilitation/Ocular Disease at the Birmingham Veterans Affairs Medical Center. Dr. Jon Forché is the only Low Vision Rehabilitation Specialist in Northeast Georgia. Dr. Forché is a vision rehabilitation consultant with the following organizations: Vision Rehabilitation Services of Northeast Georgia in Atlanta, Georgia, VISTAS in Athens, Georgia and The Vocational Rehabilitation section of the Georgia Department of Labor.
He met his wife, Dr. Nadine Forché, in Optometry School. Dr. Forché and his wife, Dr. Nadine Forché, lecture extensively to fellow optometrists about practice management, medical coding and billing. Dr. Jon Forché's hobbies include running, skiing, hiking and camping. He is a member of the American Optometric Association and the Georgia Optometric Association. He is the Treasurer/Secretary of the 10th district of the Georgia Optometric Association and the Political Action Committee Chair. He is a Fellow of the American Academy of Optometry, an organization dedicated to excellence in eye care. He spends much of his free time with his young children, Hayden and Adelyn.
In 2004 Dr. Jon Forché was honored by the Georgia Optometric Association as the Young Optometrist of the Year in Georgia. This award is presented to an outstanding optometrist, leader and volunteer in the state of Georgia. He represents the best of the optometric profession by practicing optometry with the highest standards, creating strong relationships with other health care professionals and volunteering in the community. Five Points Eye Care Center is proud of Dr. Jon Forché's accomplishments and service to the Athens community.

Nadine G. Forché, O.D., M.S., F.A.A.O.
DoctorDr. Nadine Forché is a graduate with honors from the University of Pennsylvania where she received her Bachelor of Arts degree in Neurobiology. She was class valedictorian when she received her Doctorate in Optometry from the Ohio State University College of Optometry. She also completed her Master's Degree in Physiological Optics at the Graduate School at the Ohio State University for research on the effects of Alzheimer's Disease on the visual system.
Dr. Nadine Forché completed a residency in Hospital-based and Geriatric Optometry at the Columbus Veteran's Affairs Outpatient Clinic and the Chillicothe Veteran's Affairs Medical Center in Ohio. She has lectured extensively to optometrists and optometry students and was a clinical professor at the University of Alabama at Birmingham School of Optometry. She is a pre-optometry mentor for University of Georgia students, as well as the preceptor for optometry school externs from the University of Alabama at Birmingham School of Optometry and the Pennsylvania College of Optometry at Salus University at Five Points Eye Care Center. She is a Fellow of the prestigious American Academy of Optometry and a Trustee of the Board of the Georgia Optometric Association as well as a past district President.
Much of Dr. Nadine Forché's time outside the practice is spent with her young boy-girl twins, Hayden and Adelyn (Addie), born in April 2003. Her hobbies include exercise, cooking, gardening, hiking, yoga and reading. She is a member of the American Optometric Association, the Georgia Optometric Association and the American Academy of Optometry. Dr. Nadine Forché volunteers at her children's school and is a regular blood donor. She served two terms as Treasurer for the Athens-Oconee Junior Woman's Club, a volunteer organization, and was honored as their Citizen of the Year in 2005. In 2007 her family was chosen as the Georgia Federation of Women's Clubs Family of the Year for their community involvement, leadership and family values.

Meg Brya, O.D.
DoctorDr. Meg Brya is a graduate of the University of Georgia where she received her Bachelor of Science in Education as well as a certificate in Woman's Studies. She received further teaching certification in Montessori education and taught elementary education for 7 years in Athens. She then returned to UGA to pursue pre-medical studies in preparation for optometry school while working as a vision therapist at Five Points Eye Care Center.
Graduating with honors from The Southern College of Optometry in Memphis, Tennessee, Dr. Brya continued to develop her interest in children and their visual development needs. She is a pre-optometry mentor for University of Georgia students and lectures in community on topics ranging from diabetic eye health and the role of vision in learning and development. Dr. Brya volunteers at many area schools doing vision screenings and lectures, as well as being a volunteer eye health provider with the American Optometric Association's InfantSee program and at The Mercy Clinic in Athens. Currently the director of the Vision Therapy and Clinical Departments at Five Points Eye Care Center, Dr. Brya became a partner in the practice in 2008.
A member of the College of Optometrists in Vision Development, the American Optometric Association, as well as the Contact Lens Section of the American Optometric Association, Dr. Brya is certified by Paragon Vision Sciences in the fitting of Corneal Refractive Therapy contact lenses. These gas permeable contact lenses are worn while you sleep and can allow patients to non-surgically eliminate the need for glasses and contacts during the day. An active member of professional organizations, Dr. Brya is the President of the Northeast District of the Georgia Optometric Association and was honored as their Young Optometrist of the Year in 2009. She is also a board member of the Athens Kiwanis Club. Dr. Brya is a long time resident of the Athens area. She currently resides in Watkinsville, GA with her husband and two young sons, Nicholas and Alex. She enjoys dancing, cooking, hiking, boating and relaxing with her family.

Nate Sedlacek
Licensed Dispensing OpticianNate Sedlacek joined Five Points Optical in May 2010. Nate is a Licensed Dispensing Optician and comes from a family of opticians including his father, mother and sister. He is certified by the American Board of Opticianry (ABO) and the National Contact Lens Examiners (NCLE). Nate began his career in the optical field in 1993 and has many years of experience in optical management.
Nate gives back to the community by volunteering. He traveled on a Gift of Sight trip to Mexico and volunteered on the Vision Van in Atlanta to provide eyecare to those without insurance or access to eye care. Nate lives in Winterville with his wife and 3 children and loves boating, fishing and activities around the lake.

Katie Faucher
Licensed Dispensing Optician Apprentice and Optometric TechnicianKatie joined Five Points Eye Care in November 2009 as a Licensed Dispensing Optician Apprentice and Optometric Technician. She has a Bachelor in Business Administration with a focus in Finance degree from the University of Michigan where she graduated with distinction. She was a personal shopper for Ralph Lauren for 2 years and worked at Tiffany & Co. for 3 years. She then joined Edward Jones as a financial planner, running the Troy, Michigan branch as well as training and recruiting in the region.
Katie is American Board of Opticianry (ABO) certified and passed her National Contact Lens Examiners (NCLE) examination in November 2010 in order to become a Licensed Dispensing Optician in Georgia. Katie works in both the clinic, assisting the doctors and performing pre-testing and special testing with our diagnostic equipment as well as in our optical where she helps patients choose frames and lenses that work with their lifestyle. Katie is married and lives in Athens. Her hobbies include reading, traveling and swimming.

Stacey Allen
Optometric TechnicianStacey has been an optometric technician since 1996, joining our practice in July 2000 as a general technician. Since then she has furthered her training to become proficient in all aspects of contact lenses and was promoted to Contact Lens Technician. She became certified by the National Contact Lens Examiners (NCLE) in November 2004. Stacey's responsibilities include contact lens fittings and ordering of all patient contact lenses. She also performs pre-testing, including taking patient medical history, visual acuity, blood pressure and completes other special testing as needed for the doctors. She is married to Marty Allen, an environmental protection specialist with the Environmental Protection Agency. Her hobbies include reading, photography, exercising and playing with her Doberman puppies.

Marie Isreal
Billing and Insurance ManagerMarie has been in the medical field since 1985 and joined our practice in October 2003 as the Front Office Payment Coordinator. Since then, Marie has been promoted to Insurance and Billing Manager, handling all patient billing and insurance questions and claims filing for our practice. As her proficiency in coding and billing has increased, Marie has used her knowledge to educate the doctors and staff about proper insurance coding. She is also responsible for keeping our billing software compliant with federal regulations. Marie is married with two boys, spending much of her free time with her family and friends. She also enjoys sewing, watching good movies, and reading "Oprah" magazine.

Cheryl Jones
Business Office Staff MemberCheryl joined the practice in August 2006 as a part-time business office staff member, serving as the Patient Payment Coordinator in the afternoons. She currently works with our Insurance and Billing Manager to ensure proper payment by insurance companies, following up with unpaid claims. Cheryl performs many customer service related tasks including thanking patients for choosing our office and informing patients their glasses are ready. Cheryl is an Athens native, active in her church and a caregiver for her aunt. She is married to Gary and has two adult children.

Nichole Dukes
Optometric TechnicianNichole moved to Athens from Florida and joined our practice in May of 2007 as a part-time Optometric Technician and is now full-time. She brought her past experience in the eye care field to our office, keeping our patient educational software up-to-date and organizing our exam rooms. Nichole is responsible for gathering information for the doctors, performing pre-testing and any ordered additional or special tests. She became certified by the National Contact Lens Examiners (NCLE) in November 2009. Nichole's hobbies include spending quality time with her children, being active in Kingdom Hall and baking. She lives in Oconee County and is married with two sons.

Noemi Osorio
Vision Therapist and Business Office Staff MemberAs a Vision Therapist, Noemi supervises children with various eye problems including amblyopia (lazy eye) and focusing problems as they complete vision training to improve their vision and visual function. She also works in our business office, reminding patients of their appointments and verifying insurance coverage for vision and medical benefits. Noemi joined our practice in February 2008 and lives in Athens. She is fluent in Spanish and English and is learning sign language. Her hobbies include reading, teaching, singing, and dancing.

Amye Donahoo
Optometric TechnicianAmye joined Five Points Eye Care Center in September 2004. She works full-time as an Optometric Technician. She also performs some customer service duties like reminding patients they are due for their annual or follow-up exams. She is currently studying for her National Contact Lens Examiners (NCLE) certification. Amye grew up in Athens and is married with two young sons. She is an accomplished artist and decorator, using her skills to decorate the window displays and practice for each season.

April Vaughn
Office Administrator and Business Office MemberAs our Office Administrator, April keeps our office running smoothly, working on special projects for the doctors, handling the office inventory and physical plant upkeep as well as organizing our training materials. April joined Five Points Eye Care in February 2010 and lives in Athens. She has been in the eye care field since 2005. April spends her free time playing with her puppy, volunteering at local middle and high schools as a math tutor, traveling and attending local art shows.

Karen Simmons
Business Office MemberKaren Simmons joined Five Points Eye Care in January 2011. Karen has over 16 years experience in eyecare as the business and clinic director of a retail establishment as well as a private ophthalmology practice. She is a Certified Medical Office Manager and has her National Contact Lens Examiner Certificate. Karen is multi-talented and you will see her in our business office, clinic and optical, using her extensive experience to help our patients in many areas. After work Karen enjoys spending time with her very spoiled Pomeranian and learning about her new hometown, Athens.

Natalia Williams
Business Office Member and Optometric TechnicianNatalia Williams actually started at Five Points Eye Care in 2006 but moved to Florida in 2008 when she got married. She and her husband have since moved back to the Athens area and we are very happy to have Natalia back. Natalia is a certified ophthalmic assistant and works in both our business office and clinic. She enjoys working in the same office as her sister, Noemi Osorio, our Vision Therapist. Her hobbies include decorating house, sewing and cake decorating. She enjoys spending time with her husband and their playful morkie, Mojito.

Michelle McCoy
Business Office MemberMichelle joined Five Points Eye Care Center in June 2011. She moved to Athens, GA in March of 2009 from Flagler Beach, FL. Michelle is a part of our business office staff and many times will be the one answering the phone when patients call. She does local volunteer work with persons who are deaf or hard of hearing, as well as deaf and blind and is fluent in sign language. She was able to use these skills to volunteer for 5 months in Guyana, South America. Her hobbies include hiking and running with her husband, whom she married in 2000. Her only "child" is their little Papillion named Kiwi.

Kristin Brock
Business Office MemberKristin Brock has worked in the business office of Five Points Eye Care since July of 2011. She was born in Athens and moved back for college. Her love of the community has kept her in the area even after graduating from the University of Georgia in December of 2010 with a BA in English. She brings prior experience and enthusiasm to our team and greatly appreciates the opportunity to serve and meet the needs of the Athens area.
